The Transition to Work program has been developed especially for young people and provides the support a person may need to gain long term sustainable employment. This is achieved by providing support to develop skills to gain employment. Young people and their families/advocates should be the drivers of their Transition Plans and have control over important decisions in their programs.
How will we do this?
We focus on employment outcomes and overcoming barriers that young people may face when looking for work.
Build independence by ensuring support that is focused on building the young person’s capacity to sustain work through increased independence and acknowledging developing skills.
Training is delivered in a workplace environment to assist each person with practical learning and skill development.
Working in partnership with young people, their families and local businesses.
